4 Year Direct MD:

• Completed Online Application
• Color Copy of Passport + 2 copies of Color Passport Photos
• Official Transcripts from all Institutions attended
• Personal Statement (One page or 1500 words explaining why you want to become a Physician)
• Resume or CV
• 2 Letters of Recommendation (At least one must come from an Academic source. Both letters should be dated within 1 year & contain appropriate Letterhead, Signature and Contact Information)
• MCAT Scores (if applicable)
• Application Fee of $100 USD

5 Year Combined Pre Med/MD:

• Completed Online Application
